Part of the LSF Adult Advocacy mission is to help individuals build healthier, happier, and hope-filled tomorrows. We work to provide much-needed help to the elderly, the disabled, or those suffering from chronic illness.
Protection – stepping in to protect the individual from potential abuse neglect or exploitation by completing an initial assessment to determine care needs and protect the person, property and assets
Coordination – promptly applying for benefits, coordinating services, overseeing care plans, medical appointments and any necessary treatments plans. All efforts are made to adhere to the ward’s wishes and support the least restrictive safe environment.
Advocacy – Educate the ward, their family and the community about guardianship and the alternatives. This includes facilitating the Family Guardianship Course in partnership with SCF twice a year, and providing community education to individuals and groups.

A service provided by trained staff which can include a full assessment, development and implementation of a plan of care, coordination of services, oversight of property, bill paying and just-in-case on-call assistance if family are unavailable or out of town. Services are set up under contract and can include as much or as little needed.
SERVICES INCLUDE:
- Assessing your needs and developing a customized care plan
- Regular visitations and on-site support
- Coordination of medical and dental care
- Advocacy for mental health services
- Financial management, including monthly bill payment
- Property oversight and protection of assets
- Protection and advocacy of client rights
Case management is contract based only (which is fee paying).
Unfortunately we are unable to provide financial management for people on Medicaid.
We cannot serve as rep payee for persons not under our guardianship umbrella.
If guardianship is indicated, a referral form is completed to provide as much detail as possible about the individual in need. LSF staff cannot petition the court for guardianship of an individual, but can assist any referral source through the process. If guardianship is not the answer, staff will provide advice and information about other services that might help. LSF works with several non-profit and private partners to best serve our clients, and offers guardianship advocacy.
ACCOUNTABILITY
Since guardianship is such a serious step, it is always the last resort. LSF Adult Advocacy Services aims to assist in managing cases before guardianship is needed. However, if the courts independently rule that guardianship is required, LSF is equipped to oversee these cases if appointed to do so by the court.
